Ready Home Health, Inc. (“Ready Home Health”) offers SMS text messaging for individual, service-related conversations with patients, authorized caregivers, healthcare providers, and other individuals who have consented to communicate with us by text message.

1. Types of Messages

If you consent to receive SMS messages from Ready Home Health, you may receive conversational and service-related text messages regarding:

• Scheduling and availability
• Coordination of home health visits
• Service coordination
• Responses to questions or inquiries
• Other individual communications related to Ready Home Health services

Ready Home Health does not use this SMS program to send marketing or promotional messages.

2. Consent

SMS messages are sent only after consent has been obtained. Consent may be provided verbally during a telephone or in-person conversation or through another permitted opt-in method.

Consent to receive SMS messages is not a condition of receiving services from Ready Home Health.
